Sleep disruption is rarely purely psychological. Behind many cases of chronic insomnia is a physiological imbalance that has gone unidentified, because most sleep treatments do not include a medical evaluation.

Pillar 4 of the Nidra Sleep Pathway™ begins where most sleep restoration programs do not: with a physician. Before any protocol is recommended, Dr. Brodner conducts a comprehensive evaluation to identify the physical factors contributing to sleep disruption and addresses them directly.

Thyroid Dysfunction: Both hypothyroidism and hyperthyroidism disrupt sleep architecture. Subclinical dysfunction, where lab values fall within the normal range but at its edges, can produce significant sleep symptoms frequently attributed to anxiety.

Adrenal Dysfunction and Cortisol Dysregulation: Chronic stress, blood sugar dysregulation, and adrenal fatigue can produce elevated evening cortisol that directly disrupts sleep onset.

Hormonal Imbalances: Perimenopause and menopause are among the most common causes of sleep disruption in women. Declining estrogen and progesterone have direct effects on sleep architecture and temperature regulation. Testosterone deficiency in men is similarly associated with reduced slow-wave sleep.

Nutritional Deficiencies: Magnesium deficiency is one of the most common and most overlooked contributors to insomnia. Deficiencies in vitamin D, B6, and iron are also associated with sleep disruption.

Blood Sugar Dysregulation: Nocturnal hypoglycemia is a common cause of early morning awakening. Blood glucose drops several hours after the last meal, triggering cortisol and adrenaline release that produces waking at approximately 3 AM.

Sleep-Disordered Breathing: Sleep apnea produces chronic sleep fragmentation. Dr. Brodner’s background in Otolaryngology and Head and Neck Surgery gives him a uniquely comprehensive view of upper airway anatomy and its relationship to sleep quality.

Chronic Pain and Inflammation: Pain disrupts sleep architecture directly, suppressing slow-wave sleep and producing frequent micro-arousals. Elevated systemic inflammation independently disrupts sleep quality.
