Chronic insomnia is, at its most fundamental level, a nervous system problem. Not a willpower problem. Not a mindset problem. A physiological problem rooted in the autonomic nervous system’s inability to shift from the activated, alert state of wakefulness into the calm, receptive state in which sleep becomes possible.

The autonomic nervous system operates in two primary modes. The sympathetic branch mobilizes the body for action: heart rate increases, cortisol and adrenaline are released, and the brain shifts into heightened alertness. The parasympathetic branch does the opposite. It slows the heart rate, lowers blood pressure, reduces cortisol, and creates the physiological conditions in which sleep can occur.

Sleep requires the parasympathetic system to be dominant. It cannot happen while the sympathetic system is activated.

In people with chronic insomnia, the sympathetic system is chronically elevated, even at night, even in bed, even when the person is exhausted. Brain imaging studies show that people with insomnia have measurably higher metabolic activity in regions associated with alertness and threat detection than good sleepers, not just during the day but during sleep itself. This is called hyperarousal, and it is the most common physiological feature of chronic insomnia.

This is why people who were once reasonable sleepers can develop a conditioned fear of their own bedroom. The bed becomes associated not with rest but with anxiety. The nervous system learns to treat bedtime as a threat rather than a signal for safety.
